Rolling Plains Memorial Hospital strives to meet the needs of our patients through a variety of services which complement the inpatient care offered.

The services available through our support department include:

Diabetes Outreach Program

A program designed to assist the diabetic in managing their disease through education, home monitoring, support group opportunities and team work with their private physician. The program operates from 8 to 12, Monday through Friday. For more information on our Diabetes Outreach Program, please call 325-216-6056.

Prescription Assistance Program

Rolling Plains Memorial Hospital provides qualified clients assistance in applying for low or no cost medications.

Clients must have a Primary Care Physician located in Nolan County and be income eligible.

Discharge Planning and Social Services

Provides patients with information and arrangements related to needs they may encounter in planning for a return to home after hospitalization. This may include supporting patients, families and their physicians in making decisions about necessary placement in alternate care settings after discharge.
